registry, we are always accepting applications from
qualified experiences caregivers. Please stop by and

Current references and
credentials are essential to establishing a relationship
with Douglas Health Services LLC. We check all
references. Please bring names, addresses and phone
numbers of your previous clients, registries, agencies or
employers for at least the last five years.
CNA's
and HHA's will need the following:
- Your
current professional license

Professional Liability Insurance
- Social
Security Card

Driver's License
- Proof
of automobile insurance

Current CPR certificate
- Recent
Physical (within 6 months) - must show that you are free
of any communicable disease

Current HIV training certificate
- 3
Professional References (including contact information)

II Background Screening*
Companion/Homemakers will need the following:

Background Screening - The Agency for Health Care
Administration requires that all incoming caregivers have
Level II Background Screening. Once a caregiver has
supplied all the necessary credentials, Douglas Health
Services will assist you in scheduling an appointment for a
fingerprint screening.
